I request that the repairs and maintenance set out above be carried out in my suite and for such purpose I give UCCI PROPERTIES, its employees, agents and contractors permission to enter my suite between the hours of 8:00 a.m. and 6:00 p.m. in order to carry out such repairs and maintenance notwithstanding my absence from the suite at the time such entry and repairs and maintenance are made. This consent is deemed to be consent at the time of entry for purposes of the Residential Tenancies Act and I waive any further written notice of entry for the purpose of carrying out the work required to complete the work requested. I agree to pay for any damage caused by my own willful or negligent conduct or the conduct of any Permitted Occupants, guests or invitees.
